In 1875, tuberculosis was the deadliest disease in the world, accountable for a third of all deaths. A diagnosis of TB—often called consumption—was a death sentence. Koch soon embarked on a remedy—a remedy that would be his undoing.
When Koch announced his cure for consumption, Arthur Conan Doyle, then a small-town doctor in England and sometime writer, went to Berlin to cover the event. Touring the ward of reportedly cured patients, he was horrified. Koch’s remedy was either sloppy science or outright fraud.
But to a world desperate for relief, Koch’s remedy wasn’t so easily dismissed. As Europe’s consumptives descended upon Berlin, Koch urgently tried to prove his case. Conan Doyle, meanwhile, returned to England determined to abandon medicine in favor of writing. In particular, he turned to a character inspired by the very scientific methods that Koch had formulated: Sherlock Holmes.

Technically, one supposes it went to hell before the events of the story, but they go much more to hell during it. Our heroes are lost and alone somewhere on an island in the Atlantic, their leader going slowly mad and the bodies piling up behind them. They manage, barely to get off of Guarma alive, but anyone who has gotten this far in the game must have a feeling that things aren't going to be all sunshine and roses when they return. And they're not. On your way to a mission objective in Saint Denis your character starts lurching and coughing blood, a helpful stranger helps him into a doctor's office, and good news: it's tuberculosis. Doc pulls no punches: you're dying, end of story. It's at this point in the game that the last shred of doubt is gone from your mind.

While tuberculosis-related deaths in the United States are virtually non-existent in this day and age, the disease still regularly claims over 1 million lives per year in other parts of the world, and was once a vicious killer in the US as well. Some survived, but getting tuberculosis in was more or less a death sentence for many people. And in fact, it is the disease that causes Arthur, the hardened cowboy and outlaw, to ultimately lose his life. Some may be hoping that there is a way to cure Arthur's tuberculosis and keep him as the main playable character, but unfortunately, it seems impossible. It's possible that Arthur's death is meant to mirror the death of real-life gunfighter Doc Holliday.
